- The following items should be turned in at the time of registration.
o Registration Forms (can be completed by visiting https://secure.gradelink.com/3447/enrollment
- o Emergency Contact Form
o $100.00 Registration Fee made payable to KCFS
o Birth Certificate
o Baptism Certificate (if applicable)
o Immunization Record (K-8)
o Health History Form (Kindergarten Only)The following items are due at a later date.
o Physical Release (Grades K & 6)
o Dental Release (Grades K, 3 & 7)
o Athletic Physical Release (for anyone who plans to participate in a sport)All registrations are subject to approval by the principal. No child is permitted to enter school until a
birth certificate and proof of immunizations are provided. Any student transferring to SJPII or KCHS
during the school year will be subject to a 90-day probationary period. Families are required to register
in the FACTS Tuition Program. The first tuition payment must be made by your selected payment date in
August and your balance must be paid in full by your selected payment date in May. Financial Aid forms
are released in January and are due in April. Families who apply to KCFS midyear can still complete the
financial aid forms, however, it is not a guarantee monies will be available.
